PARTICIPANT WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK I, the undersigned parent or legal guardian of the above-named minor athlete, hereby acknowledge and agree to the following: 1. Assumption of Risk. I understand that participation in football training, drills, and conditioning activities involves inherent risks of physical injury, including but not limited to sprains, strains, fractures, concussions, and other injuries. I voluntarily accept these risks on behalf of my child. 2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue Midwest Trench Academy, Coach John Przybylski, Forge Fitness, and their respective agents, employees, and volunteers from any and all claims, demands, damages, or causes of action arising out of or related to my child's participation in any MTA training session, whether caused by negligence or otherwise. 3. Medical Authorization. I certify that my child is physically fit to participate in athletic training and is not under any medical restrictions that would prevent participation. In the event of an emergency, I authorize Coach John Przybylski to seek appropriate medical attention for my child. 4. Photo/Media Release. I grant Midwest Trench Academy permission to use photographs or video of my child taken during training sessions for promotional purposes on social media and marketing materials, unless I opt out by checking the box below. 5. Acknowledgment. I have read and fully understand this waiver. I am signing freely and voluntarily, and I am the parent or legal guardian with authority to sign on behalf of the minor named above.
